Nature of Action: Under section 33(6)(c) of the Health Professions Act (the “Act”), the Registrant consented and undertook not to repeat the conduct to which this matter related and to successfully complete a College-approved course on record-keeping/charting.

Reasons: The Inquiry Committee of the College initiated an investigation under section 33(4) of the Act regarding the registrant’s charting. The registrant admitted to failing to comply with section 75(c) and the record-keeping requirements in Schedule “E” of the Bylaws by:

  • failing to generate a health care record for each patient,
  • failing to record the date of each massage therapy session, and relevant information about each massage therapy session, including clinical impressions, updated information relating to the patient’s health history, and periodic reassessment findings, in each patient’s health care record,
  • storing multiple patients’ health care records together in the same folders, and
  • failing to ensure that information in patients’ health care records was complete and current.
