Nature of Action: Pursuant to section 33(2) of the Health Professions Act, the Inquiry Committee made an order suspending the registrant’s practice.

Reasons: The registrant failed to authorize a criminal record check as required under the Criminal Records Review Act by failing to complete fingerprinting for a Vulnerable Sector (VS) check. The suspension will be in effect until such time as the registrant provides a complete and valid authorization for a criminal record check as required under the Criminal Records Review Act (including fingerprinting for a Vulnerable Sector (VS) check).
