Nature of Action: Kelly Hazeldine, RMT consented to a public reprimand under the terms of an undertaking and consent agreement that she entered with the Inquiry Committee of the College. As part of the agreement, she also agreed to an undertaking not to repeat the conduct, to pay a fine and a portion of the costs of the investigation, and to complete remedial coursework and review.

Reasons: The undertaking and consent agreement relates to an investigation initiated by the Inquiry Committee on its own motion.

As part of the undertaking and consent agreement, Ms. Hazeldine admitted to failing to comply with section 61 of the College’s Bylaws, which requires all Practising registrants to maintain professional liability insurance coverage, between December 13, 2018 and January 16, 2019 and again between January 16, 2022 and April 22, 2022. Ms. Hazeldine treated a total of over 130 patients during the periods she did not hold professional liability insurance coverage.

The Inquiry Committee is satisfied that the terms of the undertaking and consent agreement will protect the public interest.
